PRIOR TO YOUR VISIT

After booking your appointment, you will receive a confirmation email, which will include a link to complete your online intake forms in your patient portal. Please complete these forms as soon as possible before your appointment. If you have any questions about the forms, experience any issues using the patient portal, or did not receive a confirmation email, please call or text me at 518.227.1669.

LATE CANCELLATION &

NO SHOW POLICY

We ask that you give 24 hours notice if you need to reschedule or cancel your appointment. Less than 24 hours notice will result in a $50 cancellation charge.

No show for a scheduled appointment will be charged the full appointment fee. ​

More than 15 minutes past your scheduled appointment time will be considered a missed appointment.

RECOMMENDED FOR

ALL PATIENTS

Please eat a light meal or snack

1-2 hours before your appointment. 

Wear comfortable, loose clothing that can be pulled above the knees and elbows. Additional coverings such as gowns, towels, and sheets will be provided to ensure you are comfortable during your treatment.

Arrive 5-10 minutes before appointment.

FSA/HSA Credit Card

Full payment is due at the

time of treatment.

INSURANCE

We do not work with any health insurance, Workmen's Compensation, Disability, or No Fault Insurance payments. 

​

A growing number of insurance companies will reimburse for acupuncture treatments.

Consult your insurer for the terms of coverage on your policy.

​

 A superbill invoice will be given upon request to you to submit for insurance reimbursement.
